Welcome to a very exciting year of preparation for the Sacraments of First Eucharist and First Reconciliation! We hope that this will be a wonderful experience for your whole family. 

Below is the list of Parent & Child Preparation/Liturgy dates for the year. If you have participated in the Sacramental program before, this year will look a bit different. The details of the program will be outlined at the Parent Orientation/Registration Meeting on Wednesday, September 28, 2011 at 7:00pm in the Parish Hall. 

For this first meeting you will need to bring: 

• Your child’s Baptismal certificate. We will make a copy and return the original to you before you leave. 
• If your child was baptized here at St. Patrick, you do not need to bring a copy of the certificate but you need to have the exact date of the Baptism in order to complete the paperwork provided at the meeting 
• $20.00 Sacramental Preparation fee (if not already paid) 

When filling out this form, PLEASE PRINT all information as this is used for the official Sacramental recording. 

There will be sign-up sheets for Prayers of the Faithful readers, Table setting and Offertory families’ for the special liturgy celebrations. Please check with your child(ren) in advance to see if this is something they may be interested in doing. We ask that each child only be signed up for one role as we have many families we wish to include.
