Registration Instructions


Fill out both pages of the family registration form completely. There is space on one form for up to three students. This is our means of contacting you in case of emergency, please write legibly.


Fill out a medical release form for each child, it will be attached to the registration form and kept for emergencies and field trips.  This form must be complete and notarized.  You may have it notarized in the Faith Formation Office when you turn in your paperwork; Monday thru Thursday 9:30 to 3:30. 



Hayride: If your child would like to come to our Hayride please fill the form out today. This is a great opportunity to meet new friends and have a good time. The cost of the hayride is $10.00 per person.  You may also come along for the fun and help chaperone too (remember you must be trained to participate)!  If you are planning on actually going on the hayride, we will also need you to pay the $10.00.  You may return the hayride forms on October 11th at the first LOGOS and Genesis session.  We need it by then so we can get a count for the hayride at the park. Don’t forget, no medical form on file, no field trip participation.


 YOU MUST SIGN UP TO VOLUNTEER AT LEAST 2 TIMES EACH YEAR.  


In order to keep our program running smoothly we need many volunteers each 

week. It is now mandatory that all volunteers have a criminal background check and take the Protecting All God’s Children class. If you have had a background check through St. Pat’s in the past 5 years you do not need to repeat them. 


 There are two different types of volunteers needed.  

  1. Parents need to volunteer to work two times during the year.  We need volunteers to be here at 5:00 and stay until 8:45 helping wherever we need you, in the kitchen, clean-up, monitors or special projects.  We realize that some of you can’t get here until later; we can adjust your arrival time if necessary. 
  2. We need approximately 40 parents to be table leaders for all 12 sessions.  You may share a table with another parent and work out a schedule between yourselves to make sure someone is always here.  If you are not going to be here on your scheduled day, try to find a replacement if you can.  Please give us as much notice as you can if your table will not be covered on any night.


Please read and sign the Discipline Plan.  Turn it in with your other completed forms.


Please turn in your completed forms with your check made payable to St Patrick Parish in the amount of $85.00. If you paid the annual $10.00 facility fee, it may be added to your check. We have a list of paid families in the office if you are unsure if you have paid this fee for the 2012-2013 school year. 

If you are paying for the hayride you may add that amount so you only have to write one check..

Genesis & Logos meetings are scheduled for Monday nights from 6:00 - 8:30 pm. Please be sure you arrive on time for each session. We have a full agenda and will need to begin promptly at 6:00 pm.
